PGY1:

Job Overview

This one-year post-graduate training program accredited by the American Society of Health System Pharmacists (ASHP) builds on Doctor of Pharmacy (PharmD) education to develop clinical pharmacists who are responsible for medication-related care of patients with a wide range of conditions, eligible for board certification and eligible for postgraduate year two (PGY2) pharmacy residency training. Residents are prepared as pharmacists for a clinical practice that integrates direct patient care, drug distribution, teaching and management. The residency program’s purpose is achieved by utilizing the four competency areas and their associated educational goals and objectives as required by the ASHP Standard. The competency areas are: (1) patient care; (2) advancing practice and improving patient care; (3) leadership and management; and (4) teaching, education, and dissemination of knowledge

Job Description- PGY1:

Minimum Qualifications :

  1. PharmD from an ACPE accredited program.

  2. Unrestricted Pharmacist license within the first 90 days of employment.

Preferred Qualifications:

  1. Experience in drug utilization review and analysis.

  2. Experience with Electronic Medical Records.

PGY2:

Job Overview

This 12-month, post-graduate year two training program builds on PGY1 residency experiences to contribute to the development of pharmacists who will specialize in clinical or administrative pharmacy practice. The program provides residents with opportunities to function independently as practitioners by conceptualizing and integrating accumulated experience and knowledge and incorporating both into the provision of patient care or other advanced practice settings. Residents who successfully complete this PGY2 program are prepared for advanced patient care, academic, or other specialized positions, along with board certification, if available. The residency program’s purpose is achieved by utilizing the four competency areas and their associated educational goals and objectives as required by the American Society of Health System (ASHP) Standard. The competency areas are: (1) patient care; (2) advancing practice and improving patient care; (3) leadership and management; and (4) teaching, education, and dissemination of knowledge.

Job Description

Duties and Responsibilities : The duties and responsibilities listed below are intended to describe the general nature of work and are not intended to be an all-inclusive list. Other duties and responsibilities may be assigned.

  1. Upon completion of an orientation period, the resident rotates though various areas of the Department of Pharmacy (Medicine, Pediatrics, Critical Care, Cancer Care, Central Operations).

  2. Reviews and approves medication orders for all Medical Center patients. This includes weekend, holiday, evening coverage as described in departmental policies and procedures.

  3. Provides continuing education for professional staff (e.g., pharmacists, physicians, nurses).

  4. Educates other health care providers by publishing drug information articles in the pharmacy newsletter.

  5. Participates in periodic reviews of therapeutic classes of drugs and present findings to the Pharmacy and Therapeutics Committee.

  6. Prepares and presents to the appropriate committee’s evidence-based reviews of drugs requested for formulary addition.

  7. Initiates and facilitates completion of drug use evaluations.

  8. Detects and reports medication errors and adverse drug reactions in a timely manner.

  9. Provides accurate, adequate and timely drug information to professional staff.

  10. Conducts medication history interviews, reconcile patients’ medication therapies and provide drug information to patients and their families.

  11. Coordinates care to ensure safe and effective transitions from inpatient to outpatient care.

  12. Documents in the medical record patient evaluations and recommendations, medication reconciliation and medication histories.

  13. Documents pharmacist interventions according to established policies.

  14. Participates a number of longitudinal activities including, a teaching seminar and student teaching (e.g., case presentations, journal clubs, problem-based learning), and completion of a year-long residency project.

Physical Requirements:

  1. Frequent standing & walking, and lifting of 5-10 lbs.

  2. May required lifting and carrying light loads, including boxes, equipment, and stooping or kneeling

  3. Requires manual dexterity using fine hand manipulation to operate prescription medications and computer keyboard.

  4. Requires ability to see medications, computer screen, and reports.

Skills & Abilities:

  1. Ability to work independently with minimal supervision and as part of a team.

  2. Superior verbal and written presentation skills.

  3. Superior communication skills, including developing and giving presentations, both 1-1, and group.

  4. Knowledge of integrated health networks and health plan drug formularies.

  5. Willingness and ability to learn and utilize new technology and procedures that will continue to develop in their role and throughout the organization.

  6. Project and resource management skills.

  7. Creative thinking and problem-solving skills.

  8. Ability to understand importance of and respect for the confidentiality of all patient information in accordance with applicable standards and regulations.

  9. Knowledge of clinical best practices and standards for optimizing medication use.

  10. Proficiency in Microsoft Office applications, including Access, Excel, PowerPoint, and Word.

  11. Ability to work with all ages, backgrounds, ethnicities, and life experiences.

  12. A robust understanding of chronic health condition management and population management.

  13. Motivational Interviewing proficiency.

  14. Ability to prioritize and resolve critical issues efficiently and effectively.

  15. Detail oriented with strong organizational skills and multi-tasking abilities.

  16. Very strong working knowledge and proficiency with technology and business software (Microsoft Office).
