The Colorado State Veterans Community Living Center at Rifle is located on Colorado's western slope in the scenic Colorado River valley. Rifle offers a mild climate, with plenty of clean mountain air and beautiful blue skies. Amidst this setting, the State Veterans Community Living Center at Rifle provides 89 beds of skilled nursing care, including a 18-bed Special Care Unit for residents with Alzheimer's Disease or related conditions. Serving veterans, their spouse and eligible "Gold Star" parents, we are committed to caring for the whole person by addressing the personal and social needs of each resident, as well as their physical and medical requirements. All staff members of Rifle are committed to providing a life of qualify and dignity for every resident. Description of Job

This is a permanent, Full-time and/or part-time, in-person essential services designated position that requires CNA licensure and the ability to work various shifts including holidays and weekends as needed.

Please note: These working arrangements are subject to change.

May be part-time and full-time positions available.

Client Care Aides provide basic hands-on nursing care in a safe, clean, infection-controlled environment.

Essential and Important Duties:

  • Provide direct resident care including bathing, dressing, undressing, assisting in and out of bed, performing personal hygiene, feeding programs, toileting programs, range of motion, transporting, and interacting socially to meet residents' basic needs.

  • Explaining patient care before, during, and after contacts; answering call lights; and assessing basic resident care needs; updating noted resident changes; recommending routines and schedules to the IDT Team.

  • Report to the charge nurse the resident's change of condition.

  • Perform basic resident observations, vital signs, blood pressure, weight, intake and output, and basic skin assessments.

  • Provide infection control, implementing/using universal precautions, and disposal of waste.

  • Clean linen room and equipment, stock supplies, and complete supply forms.

  • Correct use of safety/positioning devices, side rails, transfer belts, proper use of equipment, attend on-going staff education programs

Special Qualifications

  • Must possess a current and active CNA license.

Minimum Qualifications

A high school diploma or GED and At least one (1) year of professional experience as a CNA or performing similar duties/assignments of a CNA

Employment history is calculated on a full-time basis (40/hrs per week). Part-time employment is calculated on a pro-rated basis to determine qualifications. Substitutions

  • A current, valid nurse aide certification (CNA) from the Colorado Department of Regulatory Agencies will substitute for the high school diploma or GED

or

  • A current, valid practical nursing license (LPN) from the Colorado Department of Regulatory Agencies or any state participating in the Nurse Licensure Compact for the nurse aide certification (CNA) can be substituted for work experience.

Preferred Qualifications

  • Knowledge and ability to appropriately carry out assigned resident care based on the residents' conditions; to use judgment and to accurately recognize, report and record relevant resident information.

  • Knowledge and ability to recognize urgent or emergent resident care situations, seek assistance of the RN and/or MD/DON, and initiate appropriate emergency interventions as directed.

  • Knowledge of quality standards, procedures, and protocols

  • Knowledge of specialized client care applicable to the work situation

  • Skill in initiating, performing and completing assigned duties in providing care to residents.

  • Ability to effectively work with an elderly patient population of varying physical, mental and emotional abilities and staff from multiple work units in the facility to achieve quality care planning and delivery

  • Ability to observe, identify and respond to the resident's needs for care, including medication, equipment-assisted care and patient/family education.

  • Superior verbal, listening and written communication skills with all health care providers, residents, families and general public.

  • Demonstrated ability to display professional conduct at all times when dealing with fellow staff members, patients, families and the general public.

  • Demonstrated ability to maintain confidentiality at all times.

  • Long-term care/nursing home experience is highly desirable

  • Effective problem solving skills

  • Experience bathing and dressing residents

  • Knowledge of modified diets

  • Ability to measure food and fluid intake and output

  • Ability to relate positively and favorably to residents and families and to work cooperatively with co-workers and supervisors

  • Ability to perform accurate measurement of vital signs, including blood pressure, temperature, pulse, weight and height

This position requires the following asconditions of employment:

  • Full Background - Name check, TRAILS and Fingerprinting

  • Must pass TB test.

  • Current & active Certified Nurse Aide License - renewal required.

  • Must possess a valid, unrestricted Colorado Driver's License.

  • Must be able to travel, in order to transport residents in state vehicles.

  • Must maintain a current CPR Card and perform CPR adequate compressions and ventilations per CPR guidelines.

  • This position is "Essential Service Designation": you will be required to report without delay or interruption to provide essential or emergency services to ensure health, safety, and welfare.

  • This position has shift work: day, evening and night shifts to include holiday and weekends as needed.

  • Employees may need to pick up on-call shifts, depending on facility needs.

  • Former State employees who were disciplinarily terminated or resigned in lieu of termination must disclose the information on the application and provide an explanation as to why the prior termination or resignation should not disqualify the applicant from the current position. Absent extraordinary circumstances, prior disciplinary termination, or resignation in lieu of termination will disqualify the applicant from future State employment with CDHS.
