Responsibilities

Provides psycho-social assessments, counseling and referral to community services and resources. Educates patients, families, and Woodland Healthcare staff. The Social Work department is a self- managed work team. The social workers are on call weekends and holidays.

• Provide psycho-social assessments for inpatient and outpatient units of Woodland Healthcare.

• Counsels patient and families for psycho social-related issues.

• Refers patient and families to community services and resources.

• Acts as a resource for education and community services for patients, families, and Woodland Healthcare staff.

• Participates in Quality Control and Process Improvement activities.

• Participates in interdisciplinary rounds and special conferences. Services on Woodland Healthcare and communitycommittees.

• Supervises MSW interns.

Qualifications

Minimum Requirements:

Experience and Education:

• A minimum of one (1) year of acute care hospital experience of a combination of hospital and other clinical experience.

• This position requires a Masters degree in Social Work.

Pay Range

$33.60 - $48.73 /hour
